An application for a change of name and/or surname can be submitted by people who were born abroad but are Cypriot citizens. For the change, the following must be submitted:

• A letter requesting the change and explaining the reasons why the change is requested.
• Affidavit before the Clerk of the Cypriot Court in a prescribed type of document.

In cases where a change of name and/or surname of a minor is requested, the Affidavit must be signed by both parents, unless, based on a court order, sole parental care has been given to one parent. In this case, a photocopy of the relevant court decision should be attached:

• Original certificate of acquisition of Cypriot citizenship or original Consular Birth Certificate.
• Photocopy of receipt for paid fees of 80 euros.
